Medication Management:
- For individuals on numerous medications we also provide poly pharmacology management and work to minimize the number and dosages of medications that you need.
Patient Education:
- We provide ongoing patient education about the disorders you are coping with, treatment options and coping skills, and any medications you are taking.
Case Management:
- We offer case management services to monitor your progress, coordinate with your physicians and help with needs such as respite care, assisted living and respite if needed.
Financial Responsibility:
- Assessments are billed at the rate of $150.00 per hour for testing, scoring and report preparation time. The fee for treatment sessions varies with the type of treatment and the length of service. If we are providers for your insurance company, we will obtain a precertification for your evaluation and/or treatment prior to evaluation or treatment. In that case, you are responsible for your deductible and copayment at the time of service, as well as for any services your insurer denies. If we are not a provider for your insurance, you may either pay via cash, check or credit card at the time of service; or if you have good credit, apply for a payment plan.
Reserved for You:
- We do not double book. You appointment time is reserved exclusively for you. For this reason, we require 48 hours of appointment cancellation or reschedule. Otherwise, you are responsible to pay for the missed session prior to your next appointment.joe;
Confidentiality:
- Any information shared with us or in our records is confidential. We do not release information to family members, friends, physicians or other agencies without your prior written consent. The exception is that we have a duty to protect and are mandated reporter. Therefore, if we know that you plan to or are hurting yourself or others, we are required to notify the proper authorities.
