1. What are your fees?

The following fees are what we bill for typical services. For more specific information click on the particular type of assessment to obtain an idea of the total cost for the assessment. Bear in mind, however, that the price on assessments may vary if you have a need for additional components such as home visits, visits to your child’s school, consultation with other professionals. In short, the prices listed under each test battery  are valid for services completed in the expected number of hours. If a case requires more time than usual (e.g., extra testing is required if patient is late, or if a more extensive report is required), extra hours are billed at the usual rate. Neuropsychological Assessment

Full evaluation of brain behavior relationships in the areas of orientation, attention, concentration, visual and auditory memory, problem solving, planning, organization. Includes full records review. Complete report with detailed recommendations for IEP and intervention outside of school.

$2500.00 -$3000.00 depending upon complexity and amount of medical and educational records to be reviewed. Payable at the time of service.  *Forensic cases are more exacting from the beginning and are billed at the higher rate of $300

 These costs are ESTIMATED costs based upon the typical length and complexity of evaluation and report, but, individuals cases may vary significantly.  For example a typical neuropsychological evaluation report might be 10-20 pages in length, but an complex one could be with detailed recommendations for home and school might be 80 pages, and would be billed accordingly.

 

TESTING SERVICES

Dementia Screening

Two Hours of Testing with Report: Attention, Concentration and Memory Screening. Tuesday or Thursday 10am - 12noon.  $400.00

ADD Screening

4 Hours of Testing with Short Report: Parent behavioral questionnaires, continuous attention tasks, intelligence screening, and measure of mental control,  with brief report for treatment planning, Wednesdays 9am-2pm.  $700.00

Full ADHD Evaluation

5 Hours of Testing with Report:  Parent and teacher behavioral questionnaires, auditory and visual performance testing, mental control testing, intelligence testing, time on task, sequential memory Wednesdays, 8am-3pm.  $1200.00

Learning Disability Screening

Kaufman Intelligence Test, and the GORT-R-4 reading test, Strop Color Word Test Wednesdays 8am-12pm $800.00

Learning Battery

Full 6 hours of Testing with Report: This includes the intelligence testing academic testing Medical and developmental history and symptom reporting is included.  The report provides diagnosis and specific academic recommendations.  $1500.00, Wednesdays 8am-3pm

Neuropsychological Battery

Full Evaluation of Brain Behavior Relationships:  Evaluation of intellectual function, attention, memory, sensory perceptual functioning, executive functions personality testing, academic performance and malingering with an extensive report.  

2 consecutive Wednesdays 8am-3pm $2500.00.  An additional $150 per hour is charged for visits to schools or places of employment.

Evaluations for the Office of Vocational Rehabilitation are customized based upon their referring issues and are paid for by the referring agency.

CPT codes (Current Procedural Terminology) are procedural codes for professional healthcare services, and they are needed to find out to what extent your insurance company will reimburse a particular procedure.   Please note that those services offered through Able2Learn are educational services. Those services do not have procedure codes and do not qualify for insurance reimbursement.  For information on those services visit www.specialeducationsupport.org.

Do you take insurance? How can I get reimbursed?

Insurance assignment is usually not accepted (except Medicare), but we can help you get reimbursed. You need to pay at the time of service, but, we do offer a payment plan if needed.   We regret that we cannot simply accept you insurance payment, but even when insurers preauthorize payment, the rates of reimbursement are very unpredictable. Please read our office policies for more detailed information.


5. How can I make an appointment?

Information on scheduling  can be found on the scheduling page. All appointment requests should be accompanied by a Policy Agreement form. Neuropsychological, ADD and LD testing require a deposit and an accompanying form that can be downloaded on the Scheduling page. Please read the instructions on that page to learn the procedure for the service you require.

6. How far in advance does one need to make an appointment?

Appointments are often available within a few weeks of contacting Mind Menders. Generally, if a detailed written report is required (e.g., for special accommodations or extended time requests), 2 weeks after testing is needed for the report to be completed. 7. What if I have to cancel an appointment?

Mind Menders  requires 48 hours notice for cancellations. Patients are expected to pay for sessions that are cancelled with less than 48 hours notice.

8. I've never been diagnosed with ADD, but I feel I have symptoms. What can I do?

Many adults with ADD have never been diagnosed, since the condition was not commonly recognized when they were children. Yet is is a myth that you outgrow ADD .  In fact, ADD can continue to affect a person's life throughout their life span.  The specific elements used in diagnosing ADD are discussed in under "How is ADD Diagnosed?" If you believe this applies to you, you should schedule an appointment with Dr. Crum  for an initial consultation to ask questions and decide if testing is right for you. The best way to define the problem is testing. The least expensive assessment offered by Mind Menders for ADD is the Brief ADD Battery . A Full ADD testing is also available.

There are a range of treatment options available to those with ADD. The first step, however, is an evaluation to confirm diagnosis and identify how the ADD is impacting your life.

9. What alternatives to medication do you offer?

Dr Crum does not prescribe medication at the present time, but does coordinate with either your private physician or Dr. Kendra Pyle to do so when necessary.  Generally, however, Dr. Crum employs effective non invasive treatment methods to help patients cope with and overcome their unique needs. Cognitive remediation - For learning disabilities and other problems in brain dysfunction, cognitive remediation is often recommended. This consists of structured exercises designed to improve weak cognitive abilities. Cognitive remediation can also be used to improve a person’s score on standardized tests, such as the SAT, ACT, GRE, LSAT, MCAT and others.

Psychotherapy - Dr. Crum is available for individual psychotherapy sessions during weekdays for those with ADD, LD, or most other psychological condition

Feedback Based Attention Training - This new treatment modality has been shown to be effective in treating attention problems from a variety of causes.

Coaching - Coaching is a collaborative relationship designed to help people achieve their goals. Usually it is a relationship that lasts over time, with regular contacts on the phone or in person in which goals, plans and the client's progress are reviewed.



10. What types of tests do you give? What are they for?

For the Brief ADD Battery, includes measures that  assess auditory and visual attention, personality, mental control and general intelligence For full evaluations  includes   full intelligence battery  as well as measures of visual and auditory attention, mental control and daily functioning. For neuropsychological evaluations include measures of attention, concentration, memory, executive functions such as organization and planning, as well as sensory perceptual functions such as visual and auditory processing.

11. How long does testing take?

This depends on the type of evaluation being done. Please click on the links below to learn more about each type of evaluation. Note that these time estimates are based on an average evaluation. If a patient requires additional testing in order to further delineate a specific deficiency and/or tests very slowly, additional time might be necessary. The following estimates are for face-to-face testing time only. An additional 90 minutes in the office is needed to fill out personality inventories. Report writing takes about 7 additional hours (for a full report).

12. Do you give a diagnosis after testing?

Upon completion of testing, Dr. Crum  gives a diagnosis in the report. However, it is important to note that as the testing becomes more extensive, more definition can be brought to the diagnosis. Therefore, it is recommended to test as much as you can afford. In the case of the Brief ADD Evaluation offered by the Mind Menders  the diagnosis offered is usually sufficient for the purpose of a patient's personal information, but would not be sufficient in requesting special accommodations or an Individualized Educational Plan. In any case where the goal is receiving extended time, special accommodations, or a well-defined, differential diagnosis, full neuropsychological assessment testing is required.  

13. Do you test children?

Dr. Crum does assessments for children, adolescents and adults.

14. Who does the testing?

Most testing is performed by both Dr. Crum, though there are some computer administered tests.
